Providence Health & Services Transition Planning Social Services II - Social Services in Everett, Washington
Providence is calling a Transition Planning Social Services II (0.78 FTE, DAY SHIFT) at Social Services of Providence Regional Medical Center Everett in Everett, WA.
We are seeking a detail-oriented licensed Social Worker to plan and coordinate activities related to transition planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. This role will require working weekends on a rotational basis.
In this position you will:
Assures individual respect for all while maintaining composure in dealing with difficult situations.
Demonstrates age specific patient care considerations for the population served.
Recognizes and assesses individual patient and family needs or requirements for safe and appropriate discharge from the acute care setting.
Leads, develops and implements appropriate discharge plans for patients transitioning from an acute care facility to the next appropriate level of care or community.
Required qualifications for this position include:
Master's Degree in Social Work or Counseling
Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting
Current Agency Affiliated Counselor, Licensed Mental Health Counselor or Associate; Licensed Advanced Social Worker or Associate; or Licensed Independent Clinical Social Worker or Associate.
WA State DOH licensure
Preferred qualifications for this position include:
- Experience in an acute hospital setting doing discharge planning is preferred.
