Providence is calling a Social Worker (MSW) (Hospice, Day Shift, 1.0 FTE) to Providence Hospice and Home Care of Snohomish County in Everett, Washington.

In this position you will:
Promote patient and family choice, assuring continuity of care and enhancing the quality of life of dying patients.
Develop and implement a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG).
Address psychosocial issues that may include adjustment to illness/prognosis, care system, community resources, insurance/financial, legal/ethical, and end-of-life planning.
Use your exceptional communication skills to help patients and families deal with the impact of end-of-life planning.
Required qualifications for this position include:
Master’s degree in Social Work (MSW) from an accredited school
Current Social Work Certification or License through Washington State Department of Health
Current Cardiopulmonary Resuscitation (CPR) certification
Valid Washington State driver’s license and daily access to a reliable, insured vehicle
1 year of experience in a healthcare setting
Preferred qualifications for this position include:
Prior social work experience in hospice or palliative care setting
2 year's of experience in a healthcare setting

Our Inpatient Hospice Care Center provides a unique alternative to traditional home-based care for hospice patients who need symptom management which cannot be provided in the home. Located at Providence Regional Medical Center Everett, the center serves patients in the hospital and offers the community a complete continuum of hospice care.
