Providence Health & Services RN Transition Coordinator in Everett, Washington


Providence is calling a Registered Nurse Transition Coordinator (0.8 FTE, Day) to Providence Regional Medical Center Everett in Everett, WA.

We are seeking a Transition Coordinator who is an experienced registered nurse (RN) to serve as a patient advocate, discharge nurse and provide follow up calls to assess and assist the patient’s transition home. The RN Transition Coordinator will make appropriate referrals for follow up care and rehabilitation and provide ongoing continuity between the patient, their surgeon and their primary care physician until the transition of care back to the PCP has been accomplished.

In this position you will:

  • Make contact, schedule and provide pre-operative teaching to surgery patients and their families that covers all aspects of care that will be provided/anticipated (ex. respiratory care, nursing, lab, physical therapy, discharge planning) in a concise, well-organized and efficient way.

  • Serve as an ongoing resource for patients and families during their diagnosis and treatment period so that they are knowledgeable about their transition pathway.

  • Create, edit and continually assess program components to ensure it meets or exceeds the guidelines set within CMS, HCAPHPS and Core Measures.

  • Screen and assess patients for post discharge complications for early intervention or prevention of readmission.

  • Be responsible for the outreach, education and support of relationships to assure ease of transition to SNF’s as required by the patients.


Required qualifications for this position include:

  • Bachelor’s Degree in Nursing (BSN) from an accredited school or program

  • Current Registered Nurse (RN) licensure in Washington State

  • Acute experience in the specialty area assigned (ex. cardiovascular CC, orthopedic surgery, post-operative care)

Preferred qualifications for this position include:

  • Master's Degree in Nursing (MSN) from an accredited school or program

  • Experience as appropriate to specialty (ex. cardiac rehabilitation, outpatient cardiac nursing care, outpatient orthopedics, physical therapy)

  • Case Management/Navigator experience

  • Specialty certification in the area of focus
