Providence Health & Services RN - Case Management Coordinator - Social Services in Everett, Washington


Providence is calling for a Case Management Coordinator RN (RN) (1.0 FTE, Day Shift) to work in Social Services at Providence Regional Medical Center Everett in Everett, WA.

The Case Management Coordinator RN is responsible for acting as a resource for assigned case management department staff in problem-solving, serves as a role model, and provides limited supervision of department staff, functions, and processes. In addition, the Case Management Coordinator RN provides case management services to a select group of assigned patients in accordance with department standards, policies, and procedures.

As a Coordinator, acts as a resource person for other employees. May train or communicate instructions to other employees, assigns and adjusts resources as needed, and inspects work to separate acceptable work from unacceptable. Serves as a contact person in the absence of a manager or director. Contributes to performance appraisals for assigned staff. This position will serve as a flex/surge position to fill in where gaps are identified in day to day operations as well.

In this position you will:

  • Work closely and collaboratively with Manager and Director to develop department objectives and strategies to maximize staff efficiencies and operations that support organizational strategies and initiatives: Collaborates with the manager and director in employee performance evaluation; Identifies and addresses clinical and operational issues that impact the provision of services with manager and director

  • Assist and develop a successful and cohesive team, with high level of productivity and accuracy to achieve performance metrics: Assures appropriate and timely response to issues that impact the provision of services.

  • Promote positive working environment for staff; understanding the importance of team-based care in service delivery: Works well with various groups of people, teams and organizations functions; Adjusts personal leadership/communication style in order to collaborate and influence with respect; Energizes others by demonstrating a positive attitude and encouraging others; Stabilizes groups by listening, staying calm, and unifying people with their words and actions

  • Oversight of the day-to-day operations of the team to ensure regulatory standards are met: Identifies clinical an operational issues and addresses them in a timely and appropriate manner- and/or escalates to manager.

  • Oversee administrative functions of the team, monitoring department productivity, reviewing department policies and procedures, and monitoring individual staff workloads/assignments: Provide expertise and knowledge of case management, case discussion and consultation; Manage complicated case management requests from patients, providers, and staff and/or escalate to manager level.

  • Assist with case management workflows and standard work processes, as well as assist with department policies and procedures: Partners with the manager and director regarding keeping processes and policies and procedures up to date; Participate and contribute to departmental meetings, team huddles, rounds, other meetings, as needed, to ensure cohesive department operations; Complete all required trainings (both online training and face-to-face training) within required timelines.

  • Perform audits of assessments, care plans and service notes to verify cases are properly managed and client coordination activities are occurring and appropriately documented: Demonstrates innovative thinking and is open to new ideas and perspectives and facilitates creativity in others; Provide data collection and feedback regarding trends and gaps of Care Management documentation/metrics.


Required qualifications for this position include:

  • Bachelor's Degree Nursing

  • 3 years experience with discharge planning and case management in an acute care setting.

  • Registered Nurse in the State of Washington.

  • WA State DOH licensure.

Preferred qualifications for this position include:

  • Master's Degree Nursing

  • Case Manager Certification or American Case Management Certification preferred.
