Providence Registered Nurse (RN) Care Specialist in Everett, Washington
Providence is calling a Registered Nurse Care Specialist (1.0 FTE, Day Shift) to the Managed Care Support Department at our Providence Medical Group Mill Creek Clinic.
The RN Specialist is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This individual works in coordination with multiple Providers and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Specialist assists patients to gain self efficacy/management skills, achieve optimum functional health status, and quality of life.
In this position you will:
Provide leadership and continuity of care guidance for patients with chronic diseases and/or complex disease states. Uses knowledge of illness, diagnoses and treatments in comprehensive, holistic assessments of patients.
Communicates with coworkers and other departments in a professional manner; shares appropriate information with all members of the health care team, including assistive/support personnel, in a manner that is appropriate and timely for the patient’s condition.
Supports vision and mission of Providence Health & Services, supports philosophy of patient centered care.
Assists in designing and implementing planned care interventions through a combination of evidence-based guideline/pathway utilization and individualized care planning
Required qualifications for this position include:
Graduate of an accredited school of nursing
Current Registered Nurse (RN) licensure in Washington State
Resuscitation Quality Improvement (RQI)
Minimum 2 years RN experience
Preferred qualifications for this position include:
Experience managing complex patients in an ambulatory setting
Medical Home Model experience
Certification of diabetes education (CDE)
