Providence Health & Services Discharge Planner Pavillion - NICU and Labor and Delivery in Everett, Washington
Providence is calling a Discharge Planner Pavillion (0.8 FTE, Day Shift) to Providence Pavillion for Women and Children in Everett, WA.
Plans and coordinates activities related to discharge planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care.
In this position you will:
Assure individual respect for all while maintaining composure in dealing with difficult situations
Demonstrate application of knowledge, skills, and behaviors in the physical, motor/sensory, cognitive, psychosocial, patient and family education areas based on PRMCE age-specific care competency assessment tool
Recognize and assess individual patient and family needs or requirements for safe and appropriate discharge from the acute care setting
Lead, develop and implement appropriate discharge plans for patients transitioning from an acute care facility to the next appropriate level of care or community.
Required qualifications for this position include:
Master's Degree / Social Work or Counseling or Bachelor's Degree in Nursing
Current agency affiliated counselor.or
Licensed Mental Health Counselor or Associate.or
Licensed Advanced Social Worker or Associate; or.Licensed Independent Clinical Social Worker or Associate; or.Registered Nurse in the State of Washington – Or.able to obtain one of these designations within 90 days of hire
Discharge planning for medical patients in an inpatient or outpatient setting.
Preferred qualifications for this position include:
- Recent or current experience discharge planning in a hospital setting
